• Before you may register, you must first have been given the all-clear by a licensed medical professional that the Polytechnic accepts. Therefore, before registering, you must have the attached Medical Examination form properly filled out by a certified Medical Practitioner and returned by you.
• In the medical center, you will need to register with Tertiary Institutions Social Health Insurance Programme (TISHIP).
• You must arrive right away for screening and registration.
• No loose, strange, inappropriate, or suggestive clothing is permitted since you must dress decently.
• Before making any payments, you must first seek departmental approval since once receipts have been issued, there will be no refund of expenses.
• Fees must be paid in full amount at the specified banks.

This is a provisional offer of admission, which means it may be withdrawn at any time. In the event that you accept this offer under the aforementioned terms, kindly report to the department for clearance.
